New Leaves Clinic

 


Wait Times

  • Learning Disability Assessment: We are scheduling intakes in the next month, and assessment in January.
  • Autism, ADHD, Sensory, Anxiety/Depression Assessment: We are currently scheduling assessments.
  • Autism/ADHD Treatment: We are not taking any new therapy clients at this time. Our wait list is closed.
  • General Counseling: We are not taking any new therapy clients at this time. Our wait list is closed.

Returning Clients

  • (Returning after 6 months) Please call our office manager to update your insurance and contact information. We will make every effort to schedule you as soon as possible.
  • Current clients, please call our office manager.

  • Our office manager will send you multiple documents that must be completed in our portal. 
  • Once all of the forms are completed (online) our office manager will call to schedule the intake appointment and take a $300 deposit.
    • Children:
      • The intake is a parent only appointment
      • Only a legal guardian can initiate medical treatment.
    • Young Adult
      • Individuals 18 and older must sign all their own documents and initiate treatment on their own. They can choose to sign a release of information to allow parent assistance.

  • Therapy: Your provider will create treatment goals and suggest a treatment schedule. You can call our office to schedule those sessions.
  • Assessment: Your provider will create a Fee Agreement for you to sign in the portal. Once you sign the fee agreement our office manager will set up an assessment date. We require a deposit of the full fee agreement to schedule.

  • Assessment: If you cancel the assessment with less than 1 week notice (or do not come on the scheduled date), we will retain $500 of the deposit and refuse further treatment. You can not cancel the second portion of an assessment, once the assessment has begun (i.e., testing over two days).
  • Intakes & Therapy Sessions: We bill by the hour, regardless of the amount of time you attend the session. Sessions from 1-53 minutes are billed at the hourly fee. You are allowed one missed appointment in 12 months without a fee. You will be billed the full hourly rate for missed appointments. The fee is not billable to insurance. We have a strict 72 hour cancellation policy.

Note: We are out of network for all insurance providers.

Good Faith Estimate

You have the right to receive a “Good Faith Estimate” explaining how much your medical and mental health care will cost.

Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the expected charges for medical services, including psychotherapy services.

You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency healthcare services, including psychotherapy services.

You can ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule a service.

If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.
